Students are able to 'pop in' or make an appointment with Helen.
For referrals with the School Based Youth Health Nurse please complete a Referral Form (PDF, 425KB) and email to nurse.mtgravatt@health.qld.gov.au (forms can be completed on line, saved and emailed)
